Document Feedback

Document Feedback is a plugin for site owners who publish documentation or help content and want to know if it actually helps readers. It appends a two-step feedback form to any post type you choose. Readers first answer whether the document answered their question. If yes, they select which details were useful. If no, they describe what they expected to find. All responses are emailed to the post author and also summarized in a meta box on the edit screen, making it easy to spot gaps or confirm what works.

• Works with any post type: By default the form appears on Pages, but you can extend it to other post types via a filter.
